Giant Retinal Tears (GRTs), particularly those exceeding two quadrants, are a surgeon's nightmare. This video demonstrates a novel approach to managing a more than 300-degree GRT with a folded and torn retina in an 8-year-old, one-eyed, intellectually disabled girl. The technique involves low vacuum vitrectomy with complete base excision with complete perfluorocarbon liquid (PFCL) fill, meticulous flattening of the torn and folded flap using a single instrument throughout the procedure, and a direct PFCL-to-heavy silicone oil exchange. By simplifying complex maneuvers and ensuring optimal retinal reattachment, this innovative method provides a reproducible solution for handling extensive GRTs. This video not only showcases a surgical breakthrough but also offers a valuable teaching tool for vitreoretinal surgeons, potentially setting a new standard for managing such challenging cases in the future.
