Impact of Trabeculectomy on Corneal Endothelium Health: A Quantitative Specular biomicroscopy study 

Study design:Cross sectional, Non interventional study 
Purpose:To assess the impact of trabeculectomy on corneal health by quantitatively analyzing changes in corneal endothelial parameters using non-invasive specular biomicroscopy.
Methods:30 primary glaucoma patients evaluated for corneal endothelial changes after trabeculectomy over 6 months. Specular biomicroscopy measured ECD, hexagonal cells, CV, and CCT preoperatively and at 1,3,and 6 months postoperatively.
Results:With female predominance, aged 40-70 years ;PACG slightly exceeded POAG. Post-trabeculectomy, reductions in ECD, CCT, and hexagonality, with increased cell area and CV. ECD loss correlated positively with CV and negatively with other parameters.
Conclusion: Trabeculectomy significantly affects corneal endothelium, causing reduced ECD, CCT, and hexagonality, with increased cell area and CV. Pre and postoperative endothelial monitoring is essential to preserve corneal health and prevent long-term visual complications.
