Quantitative OCTA Analysis of FAZ, Vessel Density, and MII in Diabetic Retinopathy (T2DM)

Objective:To determine variations in FAZ area, macular VD, and MII in various grades of DR and their correlation with different clinical parameters. Methods:A hospital-based cross-sectional study conducted in 54 eyes(28 controls, 26 diabetic,5 without DR,9 with mild,8 with moderate and 4 with severe DR)wherein OCTA measured FAZ and VD; MII was calculated. Clinical parameters(duration,HbA1c and BCVA)were analyzed along with correlation and ROC analyses. Results:Increase in FAZ area with severity was observed(P-value 0.379). VD reduced significantly from 42.88% to 29.38% (P-value 0.002).VD had strong negative correlation (Pearson’s r -0.759, P-value < 0.001) while MII had strong positive correlation (Pearson’s r 0.812, P-value < 0.001) but FAZ didn't show significance (P-value 0.127). VD had best accuracy (AUC 0.923) followed by MII (0.887) FAZ was poor (0.625). Conclusion:VD and MII are reliable markers of DR severity, with VD showing highest accuracy; FAZ has limited standalone value.
