Building Precision: An Eye Surgeon’s Journey Through 35 Botulinum Injections

Study Design: Prospective observational case series.

Purpose: To evaluate the learning curve, outcomes, and safety of periocular botulinum toxin injections performed by an ophthalmic surgeon during the first three months of practice.

Methods: Thirty-five consecutive periocular botulinum toxin injections were given for benign essential blepharospasm, hemifacial spasm, and eyelid disorders. Patients were assessed for symptomatic improvement, onset of action, dose adjustment, and complications. Injection precision and confidence were analyzed over time.

Results: Most patients improved within one week. Twenty-three patients (65.7%) had no complications. Mild transient complications in 12 patients included dry eye, ptosis, lagophthalmos, ecchymosis, and facial asymmetry. Precision improved after the first 10–15 cases.

Conclusion: Periocular botulinum toxin injections have a manageable learning curve with good safety and outcomes.
