efficacy and Safety of Botulinum Toxin Type A in Essential Blepharospasm and Hemifacial Spasm

Study Design: Prospective observational clinical study at a tertiary eye care center.
Purpose: To evaluate the efficacy and safety of Botulinum Toxin Type A (BtA) in essential blepharospasm and hemifacial spasm and identify factors associated with longer symptom-free intervals.
Methods: Twenty-six patients with primary or secondary facial dystonias receiving BtA injections between May 2022 and April 2024 were included. Severity was assessed using Jankovic Rating Scale and SMC grading. Outcomes included severity improvement, symptom-free interval, complications, and dose-response correlation.
Results: Seventy-six treatment sittings were analyzed. Significant improvement was seen at 1 week. Mean symptom-free interval was 139.92±58.53 days. Primary dystonias showed longer relief (p=0.02). Dose escalation prolonged benefit (p=0.004) without increased complications.
Conclusion: BtA is safe and effective; individualized dosing improves outcomes.
