Biometric Predictors of Angle Closure: A Case–Control Study 

Purpose:To evaluate lens-related biometric predictors of primary angle closure disease (PACD)
Methods:An observational case–control study included 80 subjects (40 PACD, 40 controls) aged 41–80 years undergoing cataract surgery (July 2022–May 2024). Lens vault (LV) and anterior chamber depth (ACD) were measured using ultrasound biomicroscopy, while axial length (AL) and lens thickness (LT) were obtained by A-scan. Relative lens vault (RLV) and lens axial factor (LAF) were also calulated.
Results: PACD eyes showed higher LT, LV, RLV, and LAF (p<0.001; large effect sizes, d>0.8) and lower ACD and AL (p<0.05). On multivariate logistic regression, RLV (OR≈4.2) and LV (OR≈3.5) were independent predictors of angle closure. ROC analysis showed superior discrimination for RLV (AUC≈0.89) compared to LV (AUC≈0.82). A combined model (RLV+ACD) further improved predictive accuracy (AUC≈0.92).Conclusion:Lens vault–based parameters, particularly RLV, are string predictors  for angle closure.
