Lost and Found

A young female with high myopia presented with retinal detachment and underwent scleral buckling with subretinal fluid drainage and transcleral cryotherapy. Preoperative evaluation revealed conjuctival lipodermoid. Intraoperatively, conjunctival peritomy revealed altered tissue planes. The inferior, lateral, and superior rectus muscles were identified and tagged; however, the medial rectus could not be located despite extensive exploration. An urgent intraoperative consultation with a strabismus surgeon was requested. Meanwhile, retinal reattachment was successfully achieved with subretinal fluid drainage and buckle placement. Further meticulous exploration along the globe and Tenon’s capsule identified residual medial rectus fibers adherent to the overlying conjunctiva. These fibers were carefully isolated, secured with sutures, and reattached to their anatomical insertion. On postoperative day 1, the patient was orthophoric, indicating successful anatomical & functional restoration.
