Methods:
This comparative step-by-step management of severe intraoperative miosis (pupil diameter less than 4.0 mm).
Few case showing  Mechanical dilation using four flexible nylon iris hooks and few cases showing Mechanical dilation using the 0.06 mm thin-profile, notch-filled B-Hex Pupil, and ease of removal
Results.
 Iris Hooks: Effective at creating a customized, wide rectangular aperture but required multiple additional corneal incisions
 B-Hex Ring: Offered a rapid, single-instrument insertion
Conclusion
While both techniques are safe but this video highlights that the B-Hex Pupil Expander offers superior surgical efficiency.
