Cataract

Stabilize, Visualize, Succeed: Cataract Surgery in Iris and Lens Coloboma

Study Design: Surgical video series.
Purpose: To demonstrate surgical modifications for safe cataract surgery in eyes with congenital iris coloboma, with and without associated lens coloboma.
Method: Three eyes underwent phacoemulsification with individualized surgical planning. Iris hooks were used in two cases, while capsular tension rings were implanted in all cases to manage zonular weakness and stabilize the capsular bag. The video highlights capsulorhexis, phacoemulsification, cortical aspiration, CTR insertion, and in-the-bag IOL implantation.
Results: All surgeries were completed successfully with stable capsular bags and well-centered IOLs. The use of iris hooks and CTRs facilitated safe surgery despite abnormal anatomy and zonular deficiency.
Conclusion: Preoperative planning and selective use of iris hooks and capsular tension rings enable safe, predictable cataract surgery in colobomatous eyes. The video shares practical surgical pearls for managing these challenging cases.
