Multilayered Internal Limiting Membrane Free Flap for Closure of Retinochoroidal Defect post Intraocular Foreign Body Removal-Novel Surgical Technique

Study Design- Single center surgical case
Purpose: Describe and evaluate the feasibility of a novel technique using a multilayered internal limiting membrane (ILM) free flap to close a full-thickness retinochoroidal defect post intraocular foreign body (IOFB) removal.
Methods: A 37-year-old man with a post-traumatic metallic IOFB embedded in the superotemporal retina underwent standard pars plana vitrectomy. After the IOFB was extracted, resulting 4×2 mm retinochoroidal crater was plugged with a multilayered ILM free flap from macula. The folded flap was stuffed into the crater under air, followed by endolaser barrage and silicone oil tamponade.
Results: The retina remained attached under silicone oil, with the crater sealed by the ILM graft. After silicone oil removal and cataract surgery, best corrected visual acuity improved to 20/40.
Conclusion: The multilayered ILM free flap is a feasible and safe option for closing a retinochoroidal defect after IOFB removal
