Navigating the Scar: Phacoemulsification in a Mature Traumatic Cataract with Adherent Leucoma Following a Self-Healed Corneal Wound

Purpose: To demonstrate the surgical management of a mature traumatic cataract with adherent corneal leucoma following a self-healed corneal wound.
Case: A patient with severe visual loss due to a mature traumatic cataract and corneal scar presented significant surgical challenges because of poor visualization and altered anterior segment anatomy.
Technique: Phacoemulsification was performed through a clear corneal incision away from the scar. Trypan blue-assisted capsulorhexis, low-fluidics phaco, careful hydrodissection, controlled nucleus fragmentation, and gentle cortical aspiration were used, followed by in-the-bag foldable IOL implantation.
Results: Surgery was completed uneventfully with successful IOL implantation and significant postoperative visual improvement.
Conclusion: Careful planning and modified phaco techniques enable safe and effective management of complex post-traumatic cataracts with excellent anatomical and visual outcomes.
