Secondary Yamane Scleral-Fixated Intraocular Lens Implantation for Long-standing Aphakia: A Surgical Video

A 60-year-old male with long-standing aphakia following cataract surgery presented for secondary visual rehabilitation. The eye had been left aphakic because of inadequate capsular support. Secondary intraocular lens implantation was performed using the Yamane double-needle sutureless scleral fixation technique. This surgical video demonstrates scleral marking, creation of angled scleral tunnels, externalization of the haptics, flange formation with low-temperature cautery, and secure intrascleral fixation, highlighting key surgical pearls to achieve optimal intraocular lens centration. The procedure was completed successfully without intraoperative complications. Postoperatively, the intraocular lens remained well centered with satisfactory visual improvement. The Yamane technique is a safe, effective, and minimally invasive option for secondary intraocular lens implantation in aphakic eyes lacking capsular support.
