18-Cut RK Catararct Surgery: Managing Wound Dehiscence and Preventing it in Fellow Eye.
Cataract surgery in post-radial keratotomy (RK) eyes presents unique challenges due to altered corneal biomechanics.We present a 56-year-old female with bilateral cataract with18-cut RK in both eyes, planned for cataract surgery with EDOF IOL. In the first eye, a posterior corneal incisions(between two RK cuts) with similarly placed side ports was created, and phacoemulsification was performed using low parameters. Despite an uneventful procedure, one RK cut opened at the end of surgery, which was closed by a horizontal suture. The patient did well in first eye. Learning from this experience, the fellow eye surgery, performed 15 days later, was modified using a scleral tunnel main incision with scleral side ports, thereby avoiding corneal stress across RK scars. This resulted in superior intraoperative wound stability, a more controlled surgical course, and excellent visual rehabilitation.
