Different Presentations of Severe Dry Eye Disease in a Private Practice Setting: A One-Year Case Series

Objective:
To describe the varied clinical presentations, etiologies, and management of severe dry eye disease (DED) encountered in a private cornea practice.

Study Design:
Retrospective observational case series.

Methods:
Records of patients with severe DED seen over one year were reviewed.

Results:
Cases included: Stevens–Johnson syndrome with rheumatoid arthritis (RA); RA factor-positive recurrent filamentary keratitis; severe DED complicated by microbial keratitis; unilateral DED following herpes zoster ophthalmicus with lid margin keratinization; drug-induced cicatrizing conjunctivitis (DICC); and RA factor-positive DED successfully rehabilitated with a scleral contact lens. Management was individualized using ocular surface optimization, anti-inflammatory therapy, infection control, and scleral lens rehabilitation.

Conclusion: Early identification of the underlying etiology and individualized management of severe DED are essential to restore ocular surface health.
