Comparative Outcomes of Implantable Collamer Lens vs Small Incision Lenticule Extraction for Low-to-Moderate Myopia:A Prospective Observational Study

Study design:Prospective Observational Study 
Purpose: To compare visual, refractive, and safety outcomes of ICL versus SMILE for low-to-moderate myopia.
Methods: In this study 200 eyes of100 patients underwent ICL or SMILE. Primary outcome was eyes within ±0.50 D of target SEQ at 6 months;secondary outcomes included UDVA, CDVA, HOAs, QoV scores,IOP, and ECC.
Results:ICL demonstrated non-inferiority to SMILE (94% vs 96% within ±0.50 D; p=0.517). Visual recovery was faster with ICL (72% vs 55% achieving UDVA ≥20/20 on Day 1; p=0.028), converging at 6 months (86% vs 88%). ICL induced significantly fewer HOAs. Mean ECC loss trended higher with ICL (6.51±13.39% vs 2.66±4.71%; p=0.060). No sight-threatening complications occurred.
Conclusions: Both procedures deliver excellent 6-month outcomes. ICL offers faster recovery and fewer HOAs; SMILE demonstrates superior endothelial preservation. Selection should be individualized based on anatomy, visual demands, and risk-benefit profiles.
