Visual and Anatomical Outcomes of Descemet Stripping Endothelial Keratoplasty In Eyes with Failed Penetrating Keratoplasty:A Retrospective Case Series
Study Design: Retrospective case series. Purpose: To evaluate the visual and anatomical outcomes of DSEK in eyes with endothelial failure following penetrating keratoplasty (PK). Methods: Ten eyes of 10 patients with failed PK underwent DSEK. Pre- and postoperative BCVA, graft clarity, central corneal thickness (CCT), and endothelial cell density (ECD) were assessed. Postoperative complications and graft survival were recorded. Results: Most eyes showed improvement in BCVA with restoration of graft clarity. CCT decreased following resolution of corneal edema. ECD showed the expected postoperative decline. Anatomical success, defined as a clear, attached graft without repeat keratoplasty, was achieved in most cases, with no primary graft failure. Conclusion: DSEK is a safe and effective alternative to repeat PK for endothelial graft failure, providing favorable visual rehabilitation and anatomical outcomes with a low complication rate.
