Title:
Navigating Uveitic Cataract: Achieving Visual Rehabilitation Through Personalized Inflammation Control and Surgical Management

Purpose
To highlight the importance of inflammation control, surgical planning and multidisciplinary perioperative care in optimizing outcomes of cataract surgery in uveitic eyes.
Methods
A 49-year-old lady with recurrent right eye iridocyclitis presented with floaters and decreased vision. BCVA was 6/24; N18 OD and 6/6(p); N6 OS. She was diagnosed with bilateral intermediate uveitis (OD>OS) with complicated cataract and CME in the right eye. Following systemic and topical corticosteroids with immunomodulatory therapy, cataract surgery was performed after maintaining disease quiescence.
Results
Postoperatively, the eye remained quiet with BCVA improving to 6/12; N6. OCT showed only trace ERM and while inflammation remained controlled on maintenance therapy.
Conclusion
Successful cataract surgery in uveitic eyes depends on sustained preoperative quiescence, careful assessment of visual potential, appropriate IOL selection, meticulous surgical technique and continued immunosuppression.
